Financial Aid Forms
The Southern Utah University Financial Aid and Scholarships office is committed to ensuring that you have a streamlined experience to receive your federal financial aid. To fulfill this goal, we use the StudentForms Verification software for you to submit required documentation and web forms to ensure the accuracy of your Free Application for Federal Student Aid (FAFSA). If you have issues navigating through the software, please review the steps in our FAFSA Verification Walkthrough video, or navigate to a specific instruction section of this page.
At times, parents will be prompted to send in additional documentation that can be submitted through the student login. Additionally, certain forms will require a parent signature to be included before submission. When you require a parent to sign the form, you may request a parent receive an email to create an account and an E-sign pin. It is important that the parent is listed on the FAFSA for this step to succeed.
Once the email is sent to the parent, they will create an account, and navigate to the section “Create an E-sign Pin”. When creating the pin, the following information must match what is explicitly listed on the FAFSA for that parent:
- First name
- Last name
- Date of Birth
- Social Security Number
Upon successful completion, the parent will be able to E-sign the form. Make sure to list the pin in a secure location for future use.
Students who have submitted their FAFSA, and have been completely admitted to 91ɬÂþ, may create a StudentForms account for FAFSA Verification. Not all students are required to complete FAFSA Verification. 91ɬÂþ will notify the student if Federal Student Aid has selected the student for verification.
First you will need to access 91ɬÂþ.Verifymyfafsa.com which will ask for your 91ɬÂþ Portal login information. After you have logged in, a new login prompt for FAFSA Verification will be provided. On this screen, you may enter credentials that you have established in previous years, or select “Create Account”. If you do not remember your past credentials, you may select “Forgot Username” or “Forgot Password” to reset them.
Select “Create Account” will take you to the account creation page. The following information that you will input must match the information you reported on your FAFSA:
- First name
- Last name
- Date of Birth
- Social Security Number
After your account is created, you will be able to login to view any required tasks, E-Sign your verification form/s, and submit additional documentation.
Students that have a FAFSA, Private Loans, or need their aid adjusted for a semester they are not attending can submit a change request form to adjust their aid.
- To request a loan select
- Select Manage Requests
- Select the appropriate Form
- 91ɬÂþ Change Request Form
- Private Loan CRF
- 91ɬÂþ Change Request Form Parent Plus
- Fill out your reason and press submit
- Select the form on the Needs Action Tab
- Select "Fill Out"
- Sign the form then Press Submit
- Finalize your form by pressing Finish
- Consortium Agreement Form 2023-24- If you are taking classes at more than one institution at the same time and wish to receive aid for credits at both institutions, you must complete this form.
- Scholarship Appeal Form - The scholarship appeal process allows students to appeal a decision regarding their institutional scholarship. The purpose of the appeal process is to provide an opportunity for students to explain the extenuation circumstances that contributed to missed deadlines or failure to meet GPA requirements. This appeal cannot be used for scholarship housing or general scholarships.
